At M2 Testing & Wellness, we are committed to providing accurate, professional, and confidential DNA testing services. By scheduling or participating in DNA testing services with our office, clients acknowledge and agree to the following policies. Identification Requirements All participants age 18 and older must present a valid government-issued photo ID at the time of collection. Minors must be accompanied by a parent or legal guardian. Additional legal documentation may be required depending on the type of test ordered. Legal & Informational Testing Clients are responsible for selecting the correct type of DNA testing. Legal DNA testing follows strict chain-of-custody procedures and may be admissible in court. Informational or peace-of-mind testing is not court admissible. Appointments Appointments are strongly encouraged. Participants should arrive on time and prepared with all required documents. Late arrivals may require rescheduling. Cancellation & Rescheduling A minimum of 12 hours’ notice is required to reschedule an appointment and 24 hours’ notice is required to cancel without a fee. Missed appointments, no-shows, or late cancellations may result in additional fees or prepayment requirements for future services. Payment Policy Payment is due at the time of service unless prior billing arrangements have been approved. We do not accept cash payments. Accepted payment methods include credit and debit cards only. Approved corporate or agency accounts may be billed according to established terms. Refund Policy DNA testing services are non-refundable once specimen collection has occurred or testing materials have been submitted to the laboratory. Refunds will not be issued for completed collections, laboratory processing, or client-requested cancellations after submission. Laboratory Processing Turnaround times are estimates only and may vary depending on laboratory processing, specimen quality, or additional testing requirements. Delays outside of our control may occasionally occur. Inclement Weather For the safety of clients and staff, we reserve the right to cancel, delay, or reschedule appointments and mobile services due to inclement weather or hazardous conditions. Privacy & Conduct All client information and results are handled confidentially in accordance with applicable privacy standards. We reserve the right to refuse service to anyone displaying disruptive, threatening, or inappropriate behavior.
